This is a control score, not an emergency severity scale.

Symptoms, sleep and daily activity matter.
A total score supports a conversation; it cannot explain every cause of breathlessness.
The five areas are scored domains, not separate diagnoses.
| 20–25 | Well controlled |

| 16–19 | Not well controlled |
| 5–15 | Poorly controlled |
Control and future attack risk are not identical. A high score does not rule out an attack.
This score companion is for people aged 12 years and older with diagnosed asthma. Complete the official adult/adolescent ACT questionnaire first. Children aged 4–11 use a different childhood test; do not apply these five-item scores or bands to that test.
No. ACT assesses recent symptom control in people with diagnosed asthma. This page totals official item scores; it is not a replacement questionnaire, diagnostic test or emergency assessment. Seek clinical evaluation for unexplained symptoms.
All five official item scores are required. A missing response is not scored as zero, averaged or estimated. The result remains blank until all five are entered.
Do not start, stop or adjust medication based on this result alone. Follow your existing clinician-provided asthma action plan and discuss the result with your healthcare professional. Seek emergency help for severe breathing difficulty, regardless of your score.
