Higher grades indicate greater activity-related breathlessness.
mMRC ≥2 indicates a higher breathlessness burden in COPD assessment. It does not determine a GOLD group or treatment by itself.

Breathlessness has many causes. Review changes with a healthcare professional.
| Grade range | What it suggests |
|---|---|
| Grades 0–1 | Lower reported breathlessness |
| Grades 2–4 | Higher reported breathlessness |
COPD assessment also needs spirometry, exacerbation history and a broader symptom review.
mMRC uses grades 0–4; the original MRC scale uses 1–5.
The grade is the selected activity description. It is not a sum, percentage or measure of oxygen saturation. The coloured categories show their order, not equal clinical intervals or a validated severity percentage.
mMRC does not diagnose COPD or asthma, determine spirometric severity, assess an acute exacerbation or prescribe treatment. Its ≥2 threshold is only part of a broader COPD symptom assessment. GOLD group assignment also requires the appropriate clinical and exacerbation history context.
Choose a statement describing your usual limitations. Do not attempt a walking or exercise challenge to choose a grade. A new or worsening change merits professional assessment; acute severe symptoms require urgent help regardless of the selected grade.
Descriptions are presented in concise third-person wording for this interface. Source references are provided below; no institutional validation of this website is claimed.
No. mMRC describes how breathlessness affects daily activity. Diagnosing COPD requires an appropriate clinical assessment and spirometry. A grade does not identify the cause of breathlessness or determine treatment.
No. Grade 0 means the selected description reports breathlessness only with strenuous exercise. It does not prove healthy lungs, normal oxygen levels or absence of disease.
This modified scale uses grades 0–4. The original MRC breathlessness scale uses 1–5. Their numbering should not be mixed. Record the scale name together with the grade.
Seek emergency help for sudden or severe breathlessness, chest pain, blue lips or other serious symptoms. Do not use a low mMRC grade to delay care. Discuss new or worsening usual symptoms with a healthcare professional.
